Yes - a Lotus 15 (chassis number '608' driven by Derek Jolly) which left the track & hit a tree during a Formula Libre race at the Melbourne Grand Prix (1958).
The engine & running gear were later transferred to a new chassis ('626') - but after taking part in the '59 Le Mans it had the '608' chassis plate fitted back (supposedly in order to avoid having to pay import duty when it was shipped back to Australia) ...
.
